Payment is expected at time of service, unless other arrangements are made in advance. Our billing staff is available to discuss any special concerns you may have. For your convenience, we accept cash, check, Visa, Mastercard, and American Express. You may also elect to have balances automatically debited from your account by filling out this form and returning it to our office.
